As far as I know, close to 99% of Klingon writers use the simple 
apostrophe used in word like "it's" and "they're", but te correct way 
for the apostrophe should be the one that looks like a 9 or a comma. You 
can write it when holding the Alt-key and type the numbers 0146. Let's 
try here: (’)
Depending on the font you use, it really looks better, but many fonts 
show it very strangely, and it might even look like the complete wrongly 
used accent: (´)

I've used that apostrophe in my exercise book, which might be visible at 
the qep'a' maybe and some of my students do have it.

Yes, you're right. Only the paq'batlh uses the simple apostrophe.

It really depends on the font, but in most cases the simple ' is more 
than enough.

Not only the dictionary section, also pages 1 to 68 use the wrong 
apostrophe. After that, it's correct.
