As SuStel has noted, we really don't have a lot of canon that uses what we'd think of as a full range of punctuation (or punctuation at all), so how you use your punctuation is really a matter of what you think will help your Klingon to make the most sense. Using a comma here is absolutely fine and not using it would also be absolutely fine.

In terms of the word order, again, as SuStel has said, it's perfect. Personally, I think of such appositions as being parallel to titles in Klingon more generally: {cheng Sa'} "General Chang", {ghawran Qang} "Chancellor Gowron", {tlha'a HoD} "Captain Klaa".

(Or {HoD Qanqor HoD}... but that's another story!)
