Perhaps we need to use a different word than "hole" in the ground. Pit, depression, trench or cavity might be better terms.
Each of them is formed by either depressing or "excavating" material from a larger whole.
Whereas "hole" in the Klingon sense is an opening "through" something.
There seems to be a distinction between a hole which is integral to the definition of an item and one that is not intended to be part of the whole.
A bullet hole, a moth hole, a tear - these are clearly not intended. Even a button hole is not necessary for the definition of a shirt
But the "holes" in a shirt for the arms and neck are necessary for the item to function as a shirt, and so might use a different word. Just as the "hole" at the neck of a bottle is what makes a bottle work - it is necessary.
